Position Summary : This is a grant funded position. Registered Nurse Case Manager/Clinical Diabetes Educator is primarily responsible for providing comprehensive diabetes education and care for patients with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and patients at high risk of developing diabetes in a clinic setting.
