Registered Nurse - Transition Team

<p>We have immediate opportunities on our Transition Team for Registered Nurses / RNs interested in helping infants, children, and adults receive complex care at home throughout Chester County, Delaware County and Montgomery County! The role is&nbsp;<strong>FT – 40 hours per week.&nbsp;</strong>This team will support the transition of care from the hospital team to ongoing home care services. This role offers a unique opportunity to learn how to care for clients with rare and complex diseases. &nbsp;As part of the Transition Team, you will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.</p> <p><strong><em>Present openings are day shifts (Monday through Sunday) and overnight shifts (Monday through Sunday).
