About the role
<p>Liquid Personnel is seeking an experienced Social Care Occupational Therapist to join a supportive and transformative Adult Social Care team in Kirklees. This role forms part of an exciting 12‑month transformation project, offering flexibility, autonomy, and the chance to make a real impact within the community.</p>
<p>You will play a key role in delivering high‑quality functional assessments, promoting independence, and supporting residents through strengths‑based, outcome‑focused practice.</p>

<li>Carry out Care Act assessments to identify needs and promote independence.</li>
<li>Complete assessments and recommendations under Disabled Facilities Grant (DFG) legislation.</li>
<li>Provide professional guidance on equipment, adaptations, and environmental modifications.</li>
<li>Work collaboratively within a supportive multidisciplinary team.</li>
<li>Maintain accurate and compliant documentation in line with statutory requirements.</li>
