About the role

<p>Our client in Leicestershire is seeking an experienced Adult Social Worker to join their Cognitive and Physical Disabilities Team. This role involves working with adults who have complex needs arising from physical disabilities, cognitive impairments, and long-term conditions. The successful candidate will be confident in managing a varied caseload, applying a strengths-based approach, and working within a multi-disciplinary environment to promote independence, wellbeing, and person-centred outcomes.</p> <p>This position requires a practitioner with strong legal knowledge, excellent assessment skills, and the ability to work proactively within a fast-paced adult social care setting.</p> <p><strong>What will your responsibilities be?</strong></p> <ul> <li>Undertaking comprehensive&nbsp;Care Act assessments, reviews, and reassessments for adults with physical and cognitive disabilities.</li> <li>Completing&nbsp;Mental Capacity Assessments, facilitating&nbsp;Best Interest decisions, and contributing to&nbsp;DoLS and community-based deprivation of liberty work&nbsp;where required.</li> <li>Developing and implementing&nbsp;person-centred care and support plans&nbsp;that promote independence, choice, and control, with a focus on strengths-based practice.</li> <li>Managing a caseload involving individuals with&nbsp;physical disabilities, learning disabilities, acquired brain injuries, and other cognitive impairments, often presenting with complex needs.</li> <li>Leading and contributing to&nbsp;adult safeguarding enquiries, ensuring appropriate risk management and safeguarding measures are in place.</li> <li>Working collaboratively with&nbsp;health professionals, occupational therapists, community services, and care providers&nbsp;to deliver coordinated, holistic support.</li> <li>Supporting&nbsp;hospital discharge pathways and community-based transitions, ensuring continuity of care and timely interventions.</li> <li>Maintaining accurate, timely case recordings and ensuring compliance with statutory responsibilities and local policies.</li> </ul> <p><strong>Qualifications &amp; Experience:</strong></p> <p>To be successful in this role you must have,&nbsp;</p> <ul> <li>Professional Qualification:&nbsp;MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.</li> <li>Registration:&nbsp;Must be registered with Social Work England.</li> <li>Experience:&nbsp;Minimum of 5 years post-qualified experience, including completion of ASYE.
