About the role
<p><span data-contrast="auto">Liquid Personnel is seeking an experienced and committed Social Worker to join their All Age Disabilities Team in Worcestershire. This role focuses on supporting young adults and adults with disabilities, delivering a seamless service into adulthood and promoting independence, wellbeing, and positive outcomes.<br></span></p>
<p><span data-contrast="auto">You will be part of a collaborative, multi-disciplinary service working with individuals whose disabilities significantly impact their daily lives, ensuring high-quality, person-centred care and support.</span></p>
<p><strong><span data-contrast="auto">What will your responsibilities be?</span></strong><span data-ccp-props="{}"> </span></p>
<ul>
<li>Work in partnership with young adults, adults, and their families/carers to assess needs and develop outcome-focused support plans.</li>
<li>Undertake statutory assessments, safeguarding activity, and care planning, ensuring wellbeing and safety is prioritised.</li>
<li>Assess risk and deliver interventions to improve quality of life and long-term independence.</li>
<li>Coordinate multi-agency services, working closely with health, education, housing, and partner organisations.</li>
<li>Support preparation for adulthood and transition planning, enabling continuity of care and independence.</li>
<li>Maintain accurate, up-to-date case records in line with statutory and data protection requirements.</li>
<li>Engage in supervision, training, and continuous professional development. </li>
</ul>
<p><strong><span data-contrast="auto">Qualifications and Experience:</span></strong><span data-ccp-props="{}"> </span></p>
<p><span data-contrast="auto">To be successful in this role you must have;</span> </p>
<ul>
<li>Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.</li>
<li>Registration: Must be registered with Social Work England.</li>
<li>Experience: Experience within a statutory setting working with young adults and/or adults, including safeguarding.</li>
<li>Knowledge: Strong understanding of relevant legislation relating to adult social care, safeguarding, and disability services.</li>
<li>Skills:
<ul>
<li>Strong assessment, analytical, and care planning ability</li>
<li>Ability to manage complex caseloads and competing priorities</li>
<li>Excellent communication and multi-agency working skills</li>
</ul>
</li>
<li>IT Skills: Experience using electronic case management systems and maintaining accurate records. </li>
