About the role

<p>Our client based in Flintshire is seeking an experienced Adult Social Worker to join their Locality Team. This is a key practitioner role reporting into the Team Manager, requiring a confident and autonomous social worker with the ability to manage complex cases and make sound professional judgements in high-risk situations.</p> <p>The successful candidate will be responsible for working with adults and their families, undertaking statutory duties, safeguarding vulnerable individuals, and delivering high-quality, outcome-focused interventions that promote independence and wellbeing.</p> <p><strong><span data-contrast="auto">What will your responsibilities be?</span></strong><span data-ccp-props="{}">&nbsp;</span></p> <ul> <li>Undertaking a range of community care and safeguarding assessments in line with statutory requirements.</li> <li>Identifying risks, needs, and appropriate interventions for adults and their families/carers.</li> <li>Managing a complex and varied caseload independently, including high-risk and sensitive cases.</li> <li>Developing and implementing outcome-focused, person-centred care plans.</li> <li>Commissioning cost-effective care packages that minimise risk and improve quality of life.</li> <li>Monitoring, reviewing, and evaluating care plans to ensure they remain effective and responsive to changing needs.</li> <li>Safeguarding vulnerable adults and making decisions regarding compulsory interventions where necessary.</li> <li>Working in partnership with individuals, families, carers, and communities to support informed decision-making.</li> <li>Building and maintaining strong relationships, providing guidance, support, and conflict resolution where required.</li> <li>Collaborating with internal teams, health professionals, and external agencies to support holistic care planning.</li> <li>Maintaining accurate and up-to-date case records and producing high-quality reports, including court documentation where required.</li> <li>Acting as an advocate for service users and carers across health and social care systems.</li> <li>Supporting the development of others, including supervising students or less experienced staff where required.</li> </ul> <p><strong><span data-contrast="auto">Qualifications and Experience:</span></strong><span data-ccp-props="{}">&nbsp;</span></p> <p><span data-contrast="auto">To be successful in this role you must have;</span> &nbsp;</p> <ul> <li>Professional Qualification:&nbsp;CQSW, CSS, DipSW, BA, BSc or MA in Social Work.</li> <li>Registration:&nbsp;Must be registered with Social Care Wales (or eligible).</li> </ul> <p><strong>Experience:&nbsp;</strong></p> <ul> <li>Minimum of 2 years post-qualified experience within Adult Social Care.</li> <li>Proven experience assessing and managing risk and delivering outcome-focused care plans.</li> <li>Experience working with adults, particularly older people, and their families/carers.</li> <li>Demonstrable experience managing complex caseloads independently.</li> <li>Experience of safeguarding vulnerable adults and undertaking related enquiries.</li> </ul> <p>&nbsp;<strong>Knowledge:&nbsp;</strong></p> <ul> <li>Strong understanding of legislation relating to Adult Social Care.</li> <li>Knowledge of safeguarding frameworks and risk management processes.</li> <li>Understanding of multi-agency working and integrated service delivery.</li> </ul> <p><strong>Skills:&nbsp;</strong></p> <ul> <li>Ability to work independently and use initiative in complex situations.</li> <li>Strong analytical, assessment, and decision-making skills.</li> <li>Excellent communication and interpersonal skills.</li> <li>Ability to produce clear, concise, and high-quality written reports.</li> <li>Strong organisational and time management
