Located in Bethpage, St. Joseph Hospital that provides comprehensive inpatient and outpatient medical, critical care and surgical services. Its Emergency Department cares for approximately 33,000 patients a year. Other vital services include an Ambulatory Surgery Unit, The Center for Sleep Medicine, Hyperbaric and Wound Healing Center, CHS Cancer Center, The Center for Speech and Swallowing Disorders, Diabetes Education Center and a full scope of outpatient radiology services, including Cardiac CT.

Job Details

Responsibilities:

• Provides medical social work services to patients and families referred by physicians and other departments. 
• Assists in discharge planning and in completion of screen instrument for patient needing placement. 
• Completes regulatory and community agencies required documentation to secure services for patients. 
• Provides information and counseling to patients and families regarding Medicare/Medicard and other entitlements.

Qualifications:

• MSW required. 
• New York State PRI Screening required. 
• Social work experience in an acute care setting strongly preferred. 

Posted Salary Range

USD $47.70 - USD $47.70 /Yr.
