Social Workers - Hospital/D2A Team

<p style="text-align: justify;">Liquid Personnel is seeking an Adult Hospital/Duty Social Worker for our client in Wolverhampton. In this role, you will be part of a fast-paced, multidisciplinary team supporting adults at key transition points in their care journey. Based within hospital settings or operating as part of the duty team, your responsibilities will include facilitating safe and timely hospital discharges, safeguarding, and promoting independence through strengths-based interventions. You will collaborate closely with National Health Service (NHS) professionals, care providers, and community partners to ensure appropriate support is in place - while upholding the principles of the Care Act 2014, personalisation and reablement.</p> <p style="text-align: justify;"><strong>What will your responsibilities be?</strong></p> <p style="text-align: justify;">In this role, you will</p> <ul> <li>Complete&nbsp;Care Act assessments&nbsp;to determine eligible care and support needs for patients ready for discharge.</li> <li>Develop and implement&nbsp;strengths-based support plans&nbsp;in collaboration with the individual, their family, and professionals.</li> <li>Work effectively within a&nbsp;multi-agency team&nbsp;including hospital discharge coordinators, nurses, OTs, and physiotherapists.</li> <li>Undertake&nbsp;safeguarding enquiries&nbsp;under Section 42 where appropriate, ensuring adult protection procedures are followed.</li> <li>Carry out&nbsp;Mental Capacity Act assessments, and contribute to&nbsp;best interest decisions&nbsp;when capacity is in question.</li> <li>Support&nbsp;timely discharge planning, ensuring that delayed transfers of care are minimised.</li> <li>Respond to&nbsp;urgent or unplanned referrals&nbsp;via the duty system, triaging and prioritising cases based on presenting risk.</li> <li>Promote the use of&nbsp;community-based resources,&nbsp;reablement, and&nbsp;preventative services&nbsp;to avoid unnecessary readmission.</li> <li>Maintain high-quality, accurate, and timely&nbsp;case records&nbsp;using systems such as&nbsp;Mosaic.</li> <li>Contribute to regular&nbsp;reviews and reassessments&nbsp;to ensure that support remains proportionate and effective.</li> </ul> <p style="text-align: justify;"><strong>Qualifications &amp; Experience:</strong></p> <p style="text-align: justify;">To be successful in this role you must have,&nbsp;</p> <ul> <li><strong>Professional Qualification</strong>: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.</li> <li><strong>Registration</strong>: Must be registered with<strong>&nbsp;</strong>Social Work England.</li> <li><strong>Experience</strong>: Substantial experience working with vulnerable adults, including those with complex needs.</li> <li><strong>Legislation Knowledge</strong>: Strong understanding of the&nbsp;Care Act 2014,&nbsp;Mental Capacity Act 2005, and safeguarding procedures.</li> <li><strong>Skills</strong>: Excellent assessment, analysis, and communication skills.
