Supervisor, Case Management

<p><strong>Crossroads Health</strong>, a 501(c)(3) non-profit organization with facilities located in Mentor, Painesville, and Cleveland, serves Northeast Ohio communities with comprehensive behavioral and primary integrated healthcare, early childhood services, extended housing and recovery services—no matter one’s ability to pay. Our mission is to provide hope, healing, and healthcare to everyone.</p> <p>We are looking at adding an organized collaborative individual to fill the role of <strong>Supervisor, Case Management-Community Services.</strong> This position offers supervision of staff and direct client care within the assigned Outpatient department. This master's level educated, licensed provider will engage as a member of a collaborative team. With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health. Position may be specific to Youth Services, Adult Services, or a combination of both.</p> <p>Responsibilities of the Supervisor, Case Management include but are not limited to:</p> <ul> <li>Provide clinical and administrative supervision of staff and program supervisor.</li> <li>Facilitate program and staff development as assigned.</li> <li>Complete ongoing performance appraisal of program staff. Assume accountability for the program’s provision of services to meet budgeted units of service and revenue targets and goals.</li> <li>Oversee implementation of assigned case management services, promoting consistent outreach and client engagement efforts.</li> <li>Provide trauma informed services to clients and their family system.</li> <li>Primary service is individual and/or family case management depending on client population, either in a primary or backup role, dependent on client/agency need.</li> </ul> <p>Position Requirements:</p> <ul> <li>Master's degree in Social Work, Counseling, or related field required.</li> <li>Minimum of 3 years clinical experience (required)</li> <li>Licensed in the State of Ohio with LSW/LPC/MFT</li> <li>Must have an acceptable driving record (4 points or less), reliable transportation, and carry automobile insurance with minimum liability limits of $100,000/$300,000 and property damage of $50,000.</li> <li>Excellent supervisory, organizational and communication skills.</li> <li>Demonstrative ability to work in an ever-changing, fast-paced environment.</li> <li>Experience utilizing data to make decisions preferred.</li> </ul> <p>Our Organization is committed to equal employment opportunity.
