This role includes conducting comprehensive assessments, developing and implementing individualized care plans, and collaborating closely with physicians and interdisciplinary team members to optimize patient outcomes.</p> <p data-start="1093" data-end="1458">You will serve as a clinical resource for staff, patients, and caregivers, offering education on prevention and management strategies, while ensuring evidence-based practices are followed. Strong assessment and critical thinking skills are essential, along with the ability to identify complex patient needs and coordinate appropriate interventions and referrals.</p> <p>Compensation: $44-55/hr.
